Clinical Rehabilitation Specialist Inpatient Rehabilitation / Rehab Work Schedule: FT Days, rotating weekends The Clinical Rehabilitation Specialist utilizes appropriate resources and communication to identify patients who may need inpatient Rehabilitation prior to returning home. Assists and coordinates the in-take and pre-admission screening process, accurately documenting evidence of patient’s compliance with established admission criteria, updates information within 48 hours prior to IRF admission. Communicates admission decisions to the referral source within 2 hours of referral receipt, and for HCA facilities, after the patient has been offered a choice of providers by case management. Performs on-site and off-site clinical evaluation to assist inpatient Rehab Medical Director in determining appropriateness of patient's for admission. Provides information related to inpatient rehab and program features to patients and family members. Communicates to patients, expectations and requirements for participation, upon admission to the rehab program. Offers each patient and family a tour of the inpatient rehab unit. Secures information relating to patient's resources and benefits. Demonstrates clinical knowledge of rehab admission criteria and advises referral sources on inpatient rehab criteria. Establishes and maintains effective working relationships with all referral sources. Completes face-to-face, in-service with physicians, case managers, other interdisciplinary members and other professional, and serves as education resource to patients, family members and caregivers. Participates in market analysis, data analysis and business plan development/ implementation for the Inpatient Rehab Unit to include visible community outreach program. Contributes to the development and maintenance of referral source information such as brochures, flyers, referrals and information kit and other outreach tools. Prioritizes meeting commitments to referral sources and develops a call-plan to prioritize utilization of time. Develops back up system to handle calls when unavailable. Develops system that ensures calls are appropriately handled 24 hours a day, 7 days a week. May assist the PAI Coordinator as necessary. Authorized to access secure drug and biological storage areas when completing a medication management orientation and when necessary to perform the assigned duties. Performs other duties as assigned.
